doc_watch: auto stale-documentation ticketing on code-without-doc commits
goal
Shipped checkouts/current/tools/doc_watch.py + --install-hook post-commit hook. Trigger: commit touches a script (.php/.py) but no doc (.md/.txt). Targeted per-doc via dir->doc map; one OPEN documentation ticket per doc, deduped by meta.doc + meta.commits. Files via jazz_telemetry.open_ticket. Verified: dry-run classification (lib->ARCHITECTURE, tools->tools/README, doc-commit->none, tagcheck->CMS_ADDITIONS), live open/skip/append against a temp db, hook install+fire, frozen-tree ignore, and self-commit filed nothing (README touched).
